Sara Elena Ramírez was born on August 31, 1975, in Mazatlán, Sinaloa, in northwestern Mexico, and relocated to the United States at age eight following their parents' divorce, settling with their mother in Tierrasanta, San Diego, California. An early aptitude for singing was identified during an audition at the San Diego School of Creative and Performing Arts, where their mother enrolled them after recognizing their musical interests. Ramírez went on to study at the Juilliard School, earning a Bachelor of Fine Arts while continuing to develop both their acting and vocal training. They speak Spanish and English fluently.
Ramírez's Broadway career began in 1998 while they were still completing their studies at Juilliard, when a casting director discovered them and offered the role of Wahzinak in Paul Simon's The Capeman, a musical based on the life of Puerto Rican gangster Salvador Agron that received negative critical notices. The following year, Ramírez appeared in Mark Lamos's The Gershwins' Fascinating Rhythm, earning an Outer Critics Circle Award nomination for their performance. In 2001, they joined the Broadway production of A Class Act, Edward Kleban's musical, taking over the role of Felecia from Julia Murney, who had originated the part off-Broadway. That same year, Ramírez also appeared in Dreamgirls, and in 2002 performed in The Vagina Monologues alongside Tovah Feldshuh and Suzanne Bertish.
The defining moment of Ramírez's Broadway career came with the 2005 musical Spamalot, written by Eric Idle and John Du Prez and based on the 1975 film Monty Python and the Holy Grail. Cast as the original Lady of the Lake, Ramírez drew widespread critical attention, with Ben Brantley of The New York Times describing them as "a toothsome devourer of scenery." The role earned Ramírez the 2005 Tony Award for Best Featured Actress in a Musical as well as the Drama League Award for Distinguished Performance.
Following their Broadway success, Ramírez transitioned to television, joining the ABC medical drama Grey's Anatomy in a recurring role as Dr. Callie Torres beginning in the show's second season. Series creator Shonda Rhimes built the character around Ramírez after meeting them, and Ramírez was elevated to star billing in the third season. They portrayed Dr. Torres for eleven seasons across 239 episodes, making the character one of the longest-running LGBT characters in American television history. Ramírez's own addition of the character's bisexuality represented one of the earliest series-regular queer roles in primetime television. The cast of Grey's Anatomy received the Screen Actors Guild Award for Outstanding Cast in a Drama Series at the 13th Screen Actors Guild Awards in 2007. Ramírez was also nominated for Outstanding Actress in a Drama Television Series at the Alma Awards in both 2007 and 2008. In addition to their dramatic work on the series, Ramírez provided a cappella vocals for "Silent Night" on the sixth-season episode "Holidaze," which aired November 19, 2009, and served as the lead vocalist for the musical episode "Song Beneath the Song," which aired March 31, 2011.
Beyond Grey's Anatomy, Ramírez portrayed Kat Sandoval on the CBS drama Madam Secretary and later played Che Díaz on And Just Like That, the Sex and the City continuation series. Both characters are bisexual and non-binary, reflecting Ramírez's own identity: after departing Grey's Anatomy, they came out publicly as bisexual and subsequently as non-binary, using they/them pronouns.
Ramírez's work in voice acting began in 1999 with the PlayStation video game Um Jammer Lammy, a guitar-based spin-off of PaRappa the Rapper, in which they voiced the title character Lammy. They reprised the role in a cameo in PaRappa the Rapper 2 in 2001, and also voiced Lammy in promotional material for the in-universe band MilkCan's album Make It Sweet! and in FMV cutscenes for the arcade version Um Jammer Lammy NOW!. Ramírez later voiced Queen Miranda in the Disney Junior animated series Sofia the First from 2012 to 2018. In music, they released the single "Silent Night" in 2009, and their self-titled EP debuted at number 37 on the Billboard 200 in 2011.
Ramírez has also been recognized for their advocacy work. Their campaigns for LGBT rights earned them the Ally for Equality Award from the Human Rights Campaign Foundation in 2015. In addition to the Tony Award, their accolades include a Screen Actors Guild Award and a Satellite Award.
