Piper Lisa Perabo was born on October 31, 1976, in Dallas, Texas, and raised in Toms River, New Jersey. Her father, George William Perabo, is a lecturer in poetry at Ocean County College, and her mother, Mary Charlotte Perabo, is a physical therapist. The eldest of three children, she has two younger brothers, Noah and Adam. Her parents named her after actress Piper Laurie. Perabo is of English, German, and Irish descent on her father's side and Norwegian descent on her mother's side. She graduated from Toms River High School North in 1994 and went on to earn a bachelor's degree in theater from Honors Tutorial College at Ohio University in 1998. During her studies, she participated in the Trinity/La MaMa Performing Arts Program in 1996 and also pursued coursework in Latin, physics, and poetry.

Her path into professional acting began while she was still a student. During a visit to New York City, casting director Denise Fitzgerald noticed her and, after learning she had no representation, made calls on her behalf that secured her an agent. After graduating, Perabo relocated to New York, where she worked as a waitress while studying acting at the La MaMa Experimental Theatre Club and taking on stage roles. Within a month of arriving in the city, she was cast in her first feature film, Marc Levin's comedy Whiteboyz.

Perabo's film breakthrough came in 2000 when she was cast as Violet "Jersey" Sanford in the romantic musical comedy-drama Coyote Ugly. The film earned over $113 million worldwide and won her an MTV Movie Award for Best Music Moment for "One Way or Another." That same year she appeared in The Adventures of Rocky and Bullwinkle as FBI agent Karen Sympathy. Following the commercial success of Coyote Ugly, she shifted her focus toward independent productions. In 2001, she starred in the Canadian drama Lost and Delirious alongside Jessica Paré and Mischa Barton, playing a boarding school student who falls in love with a female classmate. Her performance drew significant critical praise, with reviewers at the Chicago Tribune, Entertainment Weekly, and the Sacramento News & Review each citing it as a breakthrough. Roger Ebert praised her work for its sincerity and conviction.

Her subsequent film credits include a role as the eldest Baker child, Nora, in Cheaper by the Dozen (2003), a part she reprised in the 2005 sequel Cheaper by the Dozen 2. Additional films from this period include The I Inside (2003), Perfect Opposites (2004), George and the Dragon (2004), The Cave (2005), Imagine Me & You (2005), Edison (2005), and The Prestige (2006). In 2008, she co-starred with Jamie Lee Curtis in the family comedy Beverly Hills Chihuahua.

Also in 2008, Perabo made her off-Broadway debut in Neil LaBute's play Reasons to Be Pretty. The following year, in 2009, she brought that role to Broadway, marking her Broadway debut in the same production. While the play's run was concluding, she read the script for the USA Network spy drama series Covert Affairs, which led her to pursue television work for the first time. She was cast as CIA field agent Annie Walker, the lead character in the series, which ran for five seasons from 2010 to 2014. Her performance earned her a nomination for the Golden Globe Award for Best Actress in a Television Series Drama.

In September 2012, Perabo appeared in a supporting role in the time travel thriller Looper, alongside Bruce Willis, Joseph Gordon-Levitt, and Emily Blunt. Later that year she joined the sitcom Go On in a recurring role as Simone, a love interest for Matthew Perry's character, ultimately appearing in four episodes. Her subsequent television work includes a starring role in the ABC drama series Notorious (2016), a recurring role as Linda Craft in Penny Dreadful: City of Angels (2020), a role as Summer Higgins in Yellowstone from 2021 to 2024, and appearances in Billions from 2021 to 2023. In 2025, she appeared in the series Butterfly. Her film work continued with Into the Grizzly Maze (2015), Black Butterfly (2017), co-starring Antonio Banderas, Angel Has Fallen (2019), in which she played Leah Banning, and the dark comedy Spontaneous (2020). In 2025, she co-starred in the independent Quebec film Peak Everything as Tina, performing in both English and French.

In her personal life, Perabo became engaged to New Zealand director and writer Stephen Kay in 2013 and married him on July 26, 2014, in New York City. She is stepmother to Kay's daughter, Lilli. She has maintained a close friendship with actress Lena Headey since the two appeared together in The Cave and Imagine Me & You in 2005. Perabo speaks fluent French and is a part owner of Employees Only, a Prohibition-themed bar in New York's West Village that opened in 2005, and Jack's Wife Freda, a SoHo restaurant that opened in 2012.

Perabo is an advocate for LGBT rights and women's political leadership and serves on the advisory board of VoteRunLead. She is also an advisory board member for The Hometown Project, a nonprofit focused on local elections. In 2018, she was arrested while protesting Brett Kavanaugh's Supreme Court confirmation hearing, and in November 2019 she was arrested while participating in one of Jane Fonda's weekly climate change protests. She endorsed Elizabeth Warren in the 2020 Democratic presidential primaries and Kamala Harris in the 2024 presidential election, and has used her social media platforms to promote voter registration and voter education, including directing followers to VoteRiders for information on voter identification requirements.
