Pauline Angela Collins was born on 3 September 1940 in Exmouth, Devon, the daughter of Mary Honora Collins, a schoolteacher, and William Henry Collins, a school headmaster. Both parents were of Irish descent, and Collins was raised as a Catholic in Wallasey, Cheshire. Her great-uncle was the Irish poet Jeremiah Joseph Callanan. She attended Sacred Heart High School in London before training at the Central School of Speech and Drama. Prior to her acting career, she worked as a teacher until 1962.

Collins made her stage debut in A Gazelle in Park Lane in Windsor in 1962, followed by her West End debut in Passion Flower Hotel in 1965. During that production's run, she appeared in her first film, Secrets of a Windmill Girl, released in 1966. Her early television work included the UK's first medical soap opera, Emergency Ward 10, in 1960, and both the pilot episode and first series of The Liver Birds in 1969. She also played Samantha Briggs in the 1967 Doctor Who serial The Faceless Ones, having declined an offer to continue in the series as a regular companion.

Collins first achieved widespread recognition through the 1970s drama series Upstairs, Downstairs, in which she portrayed the maid Sarah Moffat across the first two series. The second series featured her husband, actor John Alderton, whom she had married in 1969. The two later appeared together in the spin-off Thomas & Sarah in 1979, the sitcom No, Honestly, and the anthology series Wodehouse Playhouse from 1975 to 1976. In connection with her Upstairs, Downstairs role, Collins recorded the 1973 single "What Are We Going to Do with Uncle Arthur?" backed with "With Every Passing Day." She and Alderton also co-narrated the animated children's series Little Miss in 1983. Collins was the subject of the television programme This Is Your Life in April 1972, when she was surprised by Eamonn Andrews.

In 1988, Collins took on the title role in the one-woman stage play Shirley Valentine in London, a performance that earned her the Laurence Olivier Award for Best Actress. She brought the role to Broadway in 1989, where it earned her the Tony Award for Best Actress in a Play, the Drama Desk Award for Outstanding Actress in a Play, and a Theatre World Special Award, all in 1989. That same year, she reprised the role in the film adaptation of Shirley Valentine, for which she received a BAFTA Award for Best Actress in a Leading Role, an Evening Standard British Film Award, and nominations for both the Academy Award for Best Actress and the Golden Globe Award for Best Performance by an Actress in a Motion Picture.

Following Shirley Valentine, Collins starred alongside Alderton in the ITV drama series Forever Green, which ran from 1989 to 1992. Her subsequent film appearances included City of Joy in 1992, My Mother's Courage in 1995, Paradise Road in 1997, and Mrs Caldicot's Cabbage War in 2002, the last of which also featured Alderton. She starred as Harriet Smith in the BBC television drama The Ambassador from 1999 to 2000. In 2005, she appeared as Miss Flite in the BBC production of Bleak House, and in 2006 she played Queen Victoria in the Doctor Who episode "Tooth and Claw," becoming the third actor to have appeared in both the original and revived series. She appeared as Mrs Gamp in the BBC series Dickensian in late 2015, and joined the cast of the comedy-drama Mount Pleasant in 2011.

Collins's personal life included a daughter, Louise, born in 1964 with actor Tony Rohr, whom she placed for adoption. She and Louise were reunited 21 years later, and Collins documented those events in her 1992 autobiography, Letter to Louise. She and Alderton had three children together and lived in Hampstead, London. Collins was appointed an Officer of the Order of the British Empire in the 2001 Birthday Honours for services to drama. In later life she was afflicted with Parkinson's disease. She died on 5 November 2025 at a care home in Highgate, London, at the age of 85.
