Maura Lynn Tierney was born on February 3, 1965, in the Hyde Park neighborhood of Boston, Massachusetts, the eldest of three children in an Irish American Catholic family. Her father, Joseph M. Tierney, served on the Boston City Council for fifteen years, and her mother, Pat, works as a real estate broker. Tierney attended Mount Alvernia Academy Elementary School and Notre Dame Academy in Hingham, Massachusetts, where drama studies led to her participation in the Boston Globe Drama Festival. She subsequently enrolled at New York University, where she began with a concentration in dance before completing a degree in drama.

Following several stage appearances, Tierney relocated to Los Angeles in 1987, making an early television appearance in Disney's made-for-TV film Student Exchange. She took on a co-starring role in 704 Hauser, a short-lived 1994 spin-off of All in the Family, before earning sustained national recognition as Lisa Miller on the NBC sitcom NewsRadio, a role she held from 1995 to 1999. Concurrent with that series, she appeared in a succession of films including Primal Fear (1996), Liar Liar (1997), Primary Colors (1998), and Forces of Nature. After NewsRadio was canceled, Tierney transitioned to dramatic television, joining the cast of ER in November 1999 in what began as a guest appearance. By February 2000, her role as Nurse Abigail "Abby" Lockhart had expanded to a full-time regular position, and the character later became a doctor following completion of medical school within the show's narrative. Tierney remained with ER for nearly ten seasons, making her final regular appearance in October 2008 and returning for a cameo in 2009 during the series' final season. Her work on the show earned her an Emmy Award nomination, which she has attributed to a story arc featuring Sally Field as Lockhart's mother. On ER, she was also reunited with her NewsRadio co-star Khandi Alexander, who held a recurring role on the drama at the same time.

During her years on ER, Tierney appeared in Christopher Nolan's 2002 film Insomnia and starred in Scotland, Pa., a film directed by her then-husband Billy Morrissette, in which she portrayed a Lady Macbeth–like character written specifically for her. Tierney had married Morrissette on February 1, 1993, having met him in 1992; the couple separated in March 2006, and their divorce was finalized after she filed in July of that year. In 2004, Tierney won the second-season tournament of Celebrity Poker Showdown, defeating Lauren Graham. She returned to the stage in May 2006 opposite Eric McCormack in the Off-Broadway premiere of Some Girl(s), and in September and October 2008 she played the role of Laurel in the Off-Broadway production Three Changes alongside Dylan McDermott.

Tierney's Broadway credits span 2008 to 2013 and include Lucky Guy and The 24 Hour Plays 2004. In 2010, she became a member of the New York City–based experimental theatre company The Wooster Group, performing alongside Frances McDormand in the company's 2010 remounting of North Atlantic, directed by Elizabeth LeCompte. From 2016 to 2019, she played feminist Germaine Greer in The Wooster Group's The Town Hall Affair, also directed by LeCompte, a production she herself conceived, which recreates a 1971 debate on Women's Liberation organized by Norman Mailer. In February 2011, Tierney made her debut at the Gate Theatre in Dublin in the Irish premiere of Yasmina Reza's God of Carnage, translated by Christopher Hampton, appearing alongside Ardal O'Hanlon. In 2019, she originated the lead role of Elizabeth in the West Coast premiere of Jen Silverman's Witch at the Geffen Playhouse, directed by Marti Lyons.

In 2009, Tierney had surgery to remove a breast tumor and underwent cancer treatments, which caused scheduling conflicts that led her to depart the cast of Parenthood, where she was replaced by Lauren Graham. She returned to work following the completion of her treatment in 2010. That same year she appeared in a recurring role on Rescue Me, returning again in 2011 during the show's final season. She also replaced Joely Richardson as the female lead in the legal drama The Whole Truth, produced by Jerry Bruckheimer, though ABC canceled the series after four episodes. In September 2012, Tierney joined The Good Wife in a recurring role as Maddie Hayward, a character who shared scenes with Julianna Margulies, with whom Tierney had previously worked on ER. In 2013, she joined the cast of the mystery drama The Affair, playing Helen Solloway, a role she held through 2019. Her performance earned her the Golden Globe Award for Best Performance by an Actress in a Supporting Role in a Series, Limited Series or Motion Picture Made for Television in 2016.

Tierney's subsequent film work includes the 2018 drama Beautiful Boy, in which she co-starred with Steve Carell and Timothée Chalamet, and The Report (2019). She appeared in Sean Durkin's wrestling drama The Iron Claw in 2023 and in Lee Isaac Chung's Twisters in 2024, the latter a sequel to the 1996 film Twister. Also in 2024, she joined the cast of Law & Order for its twenty-fourth season, portraying a new lieutenant. On television, she also joined the casts of American Rust in March 2020 and Your Honor in October 2020, and appeared in a 2011 episode of The Office as Robert California's wife.
