Jennifer Ellen Chan was born on September 16, 1958, in Harbor City, Los Angeles, California, the second of four children born to Harry Chan, a used car salesman of Chinese American heritage, and Patricia Tilly, a schoolteacher, former stage actress, and Canadian of Irish, Finnish, and Native American ancestry. Tilly has an older brother, Steve, and two younger sisters: actress and writer Meg, and artist Rebecca. Following her parents' divorce when she was five, she was raised by her mother and stepfather, John Ward, on Texada Island in British Columbia. Her mother divorced again when Tilly was sixteen, after which the family relocated to Langford, a suburb of Victoria, where she attended Belmont Secondary School before graduating from Esquimalt High School. She developed an interest in theater during her high school years, participating in plays with her mother's encouragement, and went on to earn a bachelor's degree in theater from Stephens College in Missouri.

Tilly began her professional acting career in the early 1980s, taking on small television roles that included a recurring guest part on Hill Street Blues as Gina Srignoli, a mobster's widow who becomes romantically involved with detective Henry Goldblume. She also appeared as Frasier Crane's date Candi Pearson in the fourth season of Cheers and reprised a similar character in the eleventh season of Frasier. Additional early television work included a role as Garry's girlfriend on It's Garry Shandling's Show on Showtime and a part in the short-lived comedy Key West alongside Fisher Stevens. In 1989, she appeared in Let It Ride, starring Richard Dreyfuss, and secured a prominent role in The Fabulous Baker Boys, a part written specifically for her by screenwriter Steve Kloves.

Her feature film breakthrough came with a supporting role as Olive Neal, a hopelessly bad actress, in Woody Allen's 1994 comedy Bullets Over Broadway, for which she received a nomination for the Academy Award for Best Supporting Actress, ultimately losing to her co-star Dianne Wiest. Two years later, she earned further acclaim playing Violet, a lesbian femme fatale, opposite Gina Gershon in the Wachowskis' Bound (1996). In 1997, she played Samantha Cole in the Jim Carrey comedy Liar Liar, and in 2001 she portrayed gossip columnist Louella Parsons in Peter Bogdanovich's historical drama The Cat's Meow. Her other film credits include Made in America (1993), Music from Another Room (1998), The Haunted Mansion (2003), in which she portrayed Madame Leota, Home on the Range (2004), and Tideland (2005).

Tilly achieved widespread recognition through her portrayal of serial killer Tiffany Valentine in the Child's Play franchise, a role she first took on in Bride of Chucky (1998). She reprised the character in Seed of Chucky (2004), Curse of Chucky (2013), Cult of Chucky (2017), and the Syfy/USA television series Chucky, which ran from 2021 to 2024. In Seed of Chucky and the second season of Chucky, she played a dual role, voicing Tiffany while also portraying an exaggerated version of herself. The performance in the franchise earned her nominations for the Fangoria Chainsaw Award for Best Actress and the MTV Movie Award for Best Scared-As-Shit Performance. Since 1999, she has voiced Bonnie Swanson, the Griffin family's neighbor, on the Fox animated sitcom Family Guy. Her voice work extends to the films Stuart Little (1999), Monsters, Inc. (2001), and Home on the Range (2004), as well as the children's series Hey Arnold.

On stage, Tilly received a Theatre World Award in 1993 for her performance in the off-Broadway play One Shoe Off. Her Broadway career spans from 2001 to 2012. In 2001, she starred as Crystal Allen in the Broadway revival of The Women, and in 2012 she returned to Broadway in Don't Dress for Dinner, playing the mistress Suzanne, a role that earned her an Audience Choice Award nomination. In 2013, she appeared in the Wallace Shawn and Andre Gregory collaboration Grasses of a Thousand Colors, which had premiered at the Royal Court Theatre in London in 2009 before moving to the Joseph Papp Theater in New York.

Away from acting, Tilly established herself as a competitive poker player. Her father was a gambler and poker player who introduced her to the game through a video version, and she later learned to play the real game after moving to Hollywood. On June 27, 2005, she won a World Series of Poker bracelet and $158,625 in the Ladies' No-Limit Texas Hold 'em event, defeating 600 other players and becoming the first celebrity ever to win such a tournament. On September 1 of that same year, she won the third World Poker Tour Ladies Invitational Tournament, held at the Bicycle Casino in Bell Gardens, California. She was nominated for PokerListings' Spirit of Poker Living Legend Award in 2014, and as of 2019 her live tournament winnings exceeded $1 million. In addition to her Saturn Award and GLAAD Award, Tilly has received two MTV Movie Award nominations. In 2024, she was announced as a friend of the housewives on the fourteenth season of The Real Housewives of Beverly Hills.
