Elise Demetria Neal was born on March 14, 1966, in Memphis, Tennessee, to a nurse and a construction worker. She trained as a ballet dancer and was a member of the Tennessee Ballet Company, and also participated in cheerleading during her youth, sustaining a left wrist injury from a high toss stunt that left the wrist permanently unable to straighten fully. She completed her secondary education at Overton High School for the Creative and Performing Arts before enrolling at the University of the Arts in Philadelphia, where she studied from 1984 to 1988. After two years she left college and relocated to New York City, where she pursued musical theater and toured internationally with various companies.

Neal's Broadway career spanned 1986 to 1990 and included appearances in the musicals Oh, Kay! and Uptown...It's Hot! Her foundation in musical theater facilitated a transition into commercial work, which eventually brought her to Los Angeles.

In the early 1990s, Neal worked as a dancer in numerous hip hop and R&B music videos. Among her earliest credits in this area was a background dancing role in Chubb Rock's video for "Just the Two of Us." She also appeared in Black Sheep's 1992 video for "Strobelite Honey," Father MC's 1991 video for "Lisa Baby," Dannii Minogue's 1993 video for "This Is It," Vanessa Williams' video for "Work to Do," Trick Daddy's 2001 video "I'm a Thug," and Aretha Franklin's 1998 video for "A Rose Is Still a Rose." Her film debut came in Spike Lee's Malcolm X in 1992. She subsequently took on the role of Janice Sinclair on the soap opera Loving and later portrayed fighter pilot J.J. Fredericks on the television series seaQuest 2032.

Neal gained wider recognition in 1997 through roles in three films: Rosewood, in which she played a school teacher named Scrappie; Money Talks; and Scream 2, in which she was cast as Hallie. Her character in Scream 2 had originally been written as one of the film's killers, but a script leak prompted a revision of the role. That same period saw her appear in Restaurant (1998), playing Jeanine, a waitress with aspirations of becoming a singer, and in a 2001 television remake of Brian's Song as the wife of Gale Sayers.

Her most prominent television role came when she was cast as Yvonne Hughley, the wife of D.L. Hughley's character, in the ABC/UPN sitcom The Hughleys, which ran from 1998 to 2002. The performance earned her two NAACP Image Award nominations for Outstanding Actress in a Comedy Series. Following that series, she joined All of Us as kindergarten teacher Tia Jewel, a role she held until 2005. Additional television credits include guest appearances on Law & Order, Hangin' with Mr. Cooper, and Method & Red, as well as a recurring role as Ayana Boulet on K-Ville.

Neal's film work continued with Mission to Mars (2000) and Paid in Full, and she appeared in the ensemble cast of Hustle & Flow (2005) as Yevette, the wife of Anthony Anderson's character. That performance brought her a Screen Actors Guild Award nomination for Outstanding Performance by a Cast in a Motion Picture, as well as a third NAACP Image Award nomination, this time in the category of Outstanding Supporting Actress in a Motion Picture. Her 2007 film 4 Life also marked her first soundtrack single, "I'm Down Baby." That same year she was featured in the November 2007 edition of King Magazine.

Neal played Alana in Love Ranch in 2010 and portrayed singer Gladys Knight in the television biopic Aaliyah: The Princess of R&B in 2014. In 2017 she appeared as Kathryn Munson in the superhero film Logan and also appeared on the television series Into the Dark. She has additionally been seen on the reality series Hollywood Divas and guest starred on The Soul Man. Beyond performing, Neal has pursued producing, developing projects including a reality show centered on her R&B group Assorted Flavors, a fitness and lifestyle brand, and multiple feature projects through a development deal with Mandalay Bay Entertainment.
