Barbara Ann Loden was born on July 8, 1932, in Asheville, North Carolina, and raised by her maternal grandparents in Marion, North Carolina, following her parents' divorce in her early childhood. Her father was a barber, and she described her upbringing in the Appalachian Mountains as emotionally impoverished, recalling that she had no childhood friends and spent much of her time alone. She attended high school in Asheville before moving to New York City at age 16, funding the trip with money she had received from a car accident settlement.

In New York, Loden supported herself through modeling for detective and romance magazines and working as a dancer at the Copacabana nightclub. She went on to study at the Actors Studio, of which she became a lifetime member, though she initially doubted she could succeed as an actress due to her accent. During the mid-1950s, she became a recurring presence on The Ernie Kovacs Show, where her first husband, television producer and film distributor Larry Joachim, had helped her secure a position. Her role on the program involved appearing in bathing suits and performing physical comedy stunts, work she later reflected on with ambivalence. She and Joachim had a son, Marco.

Loden made her New York theater debut in 1957 in Compulsion, launching a Broadway career that spanned from 1957 to 1964. Her stage credits include The Changelings, The Long Dream, Look After Lulu, But for Whom Charlie, and After the Fall, among other productions. She also appeared on stage in The Highest Tree alongside Robert Redford and in Night Circus with Ben Gazzara.

Her most celebrated stage role came in the 1964 Lincoln Center Repertory Company premiere of Arthur Miller's After the Fall, directed by Elia Kazan, in which she portrayed Maggie, a character understood to be a fictionalized version of Marilyn Monroe. Loden recalled in 1980 that she was drawn to the role because the script reflected her own life experiences, and she attributed her performance to five years of acting classes at the Actors Studio. The role earned her both a Tony Award for Best Featured Actress in a Play and a Theatre World Award, both in 1964, as well as an award from the Outer Circle, an organization of writers covering Broadway for national magazines.

Her film work began with a role in Elia Kazan's Wild River in 1960, followed by Splendor in the Grass in 1961, in which she played Warren Beatty's sister. She had been cast in a scene for Frank Perry's The Swimmer starring Burt Lancaster, but the scene was entirely reshot with Broadway actress Janice Rule replacing her during post-production disputes; Loden received no screen credit. Loden married Kazan, who was 23 years her senior, in 1966, and they had a son, Leo. Though the two were estranged and considering divorce, they remained married at the time of her death.

In 1970, Loden wrote, directed, and starred in the independent film Wanda, which won the International Critics Award at the Venice Film Festival that year. Throughout the 1970s, she continued directing Off-Broadway and regional theater productions and completed two short films. In 1978, she was diagnosed with breast cancer. Barbara Loden died on September 5, 1980, at the age of 48.
